IDEXX SmartFlow is a cloud-based patient workflow platform designed to digitize and track every step of in-hospital patient care — from admission through treatment, anesthesia, surgery, and discharge. Acquired by IDEXX in July 2018, SmartFlow fills a specific gap in the veterinary software stack: it is not a practice information management system (PIMS) itself, but a clinical workflow layer that sits alongside a PIMS and handles the paper-intensive processes that PIMS platforms typically do not manage well.
SmartFlow integrates with Cornerstone, Neo, and ezyVet (all IDEXX-owned PIMS platforms) and is used by thousands of veterinary hospitals worldwide. It holds a 4.5 out of 5 ease-of-use rating across 24 reviews on GetApp and is particularly valued by emergency hospitals, specialty practices, and busy general practices with significant hospitalization caseloads.

What SmartFlow actually does
SmartFlow is not a PIMS. It does not handle appointment scheduling, client communication, billing, or inventory management on its own. It is a clinical workflow overlay that manages six specific in-hospital processes:
1. Digital treatment sheets
Paper treatment sheets are one of the most failure-prone processes in veterinary hospitals. They get lost, are illegible, cannot be accessed simultaneously by multiple team members, and do not automatically connect to the invoice. SmartFlow replaces them with digital treatment sheets that:
- Allow customizable templates matched to your practice's treatment protocols
- Support on-the-fly adjustments to treatment plans during hospitalization
- Include built-in medication, diet, and fluid calculators to reduce dosing errors
- Transfer estimates into trackable treatment items so that charge changes are visible in real time
- Can be accessed from any connected device — multiple team members can view and update the same patient simultaneously
The charge-capture integration with ezyVet is particularly notable: SmartFlow claims that practices can recover up to 14% of revenue leakage from missed charges through the two-way sync between SmartFlow treatment sheets and ezyVet invoicing.
2. Electronic whiteboard
The SmartFlow whiteboard provides a real-time, bird's-eye view of every patient in the hospital. It shows:

- Upcoming treatments and procedures
- Assigned doctor and technician

4. Dental charts
SmartFlow dental charts replace paper dental records with:
- Before-and-after photo capture integrated into the chart
- Standardized dental charting across the practice regardless of which veterinarian is performing the procedure
- Patient-side charting on iPad during dental procedures
- One-click export of dental findings to the patient record
5. Digital consent forms and intake
SmartFlow digitizes consent forms, allowing clients to review and sign documents from anywhere in the practice — on an iPad at the front desk, in the exam room, or at checkout. Forms are fully customizable based on visit type and include photo capture for patient identification and personal belongings documentation.
6. Analytics
SmartFlow Analytics provides practice-level data on treatment template utilization, turnaround times, and workflow bottlenecks. This helps practice managers identify where patients are getting stuck — between admission and first treatment, between surgery and recovery, or at discharge — and make operational adjustments.

SmartFlow pricing is subscription-based, quoted per practice. The published starting price is approximately $155/month (per feature), though actual costs vary based on practice size, number of iPad stations, and whether SmartFlow is bundled with other IDEXX software products.

Hidden costs to plan for
- iPad hardware. SmartFlow is designed for patient-side iPad use. A typical hospital might need 3–8 iPads depending on size. At $300–$800 per iPad, this is a meaningful upfront investment.
- MDM (mobile device management). If IDEXX provides managed iPads, updates are handled through the App Catalog rather than the Apple App Store. Practices managing their own iPads need an MDM solution for updates and security.
- Training time. While SmartFlow is rated 4.5/5 for ease of use, transitioning from paper treatment sheets to digital workflows requires protocol standardization that takes staff time to develop and internalize.
- Connectivity dependency. SmartFlow requires internet access. If your hospital has Wi-Fi dead zones, expect to invest in network infrastructure before deployment.
PIMS integration: the critical constraint
SmartFlow's primary integration depth is with IDEXX-owned PIMS platforms, with a limited integration available for one non-IDEXX system:

SmartFlow was acquired by IDEXX in 2018 to strengthen its software ecosystem, and the deepest integrations remain with IDEXX-owned platforms. The Provet Cloud integration, added in 2025, allows sending patients to SmartFlow, tracking treatments, and discharging with automatic document attachment back into Provet. However, the charge-capture and invoicing sync available with Cornerstone, Neo, and ezyVet is not available with Provet Cloud. If your practice uses a PIMS not listed above and wants digital treatment sheets, alternatives include:
- Instinct EMR, which offers its own digital treatment sheets and anesthesia tracking and integrates with any PIMS

What SmartFlow does well
- Eliminates paper chaos. The single most commonly cited benefit in user reviews. Digital treatment sheets cannot be lost, are always legible, and are accessible to the whole team simultaneously.
- Automatic charge capture. Treatment items flow directly into the PIMS invoice, reducing the missed-charge revenue leakage that plagues paper-based hospitals. The ezyVet integration claims up to 14% recovery.
- Real-time hospital visibility. The electronic whiteboard gives every team member instant visibility into patient status, location, and upcoming treatments — from any workstation.
- Standardization across doctors and technicians. Template-based treatment sheets and dental charts ensure consistent documentation regardless of which team member is assigned to the patient.
- Built-in calculators. Medication, diet, and fluid calculators reduce manual dosing math and the errors that come with it.
- Anesthesia documentation quality. Digital anesthetic sheets with automated timers and one-touch vital-sign charting produce records that are more complete, more legible, and more defensible than paper alternatives.
- iPad-native design. The SmartFlow iPad app is optimized for patient-side use during procedures, with a touch-first interface that works with wet or gloved hands.
Where SmartFlow falls short
- Limited non-IDEXX integration. The deepest integrations are with IDEXX-owned PIMS platforms (Cornerstone, Neo, ezyVet). A Provet Cloud integration was added in 2025 for patient flow and document attachment, but charge-capture sync is not available. Practices using Shepherd, Digitail, DaySmart Vet, AVImark, or other non-IDEXX systems have no integration pathway.
- Not a PIMS. SmartFlow does not replace your practice management software. It is an additional subscription on top of your PIMS, adding to the total software stack cost.
- Requires iPads. The platform is designed for iOS tablets. While web access exists, the optimal experience requires iPad hardware investment.
- Internet dependency. SmartFlow requires consistent internet connectivity. Unlike Instinct (which offers offline mode), SmartFlow cannot operate during network outages. For practices with unreliable connectivity, this is a risk.
- No AI features. As of mid-2026, SmartFlow does not include AI-assisted documentation, predictive analytics, or clinical decision support. It is a workflow automation tool, not an AI tool. Practices wanting AI-powered clinical documentation should look at Instinct EMR (ScribbleVet AI Scribe) or Shepherd (TranscribeAI).
- Analytics are descriptive, not prescriptive. SmartFlow Analytics shows what happened (treatment times, bottlenecks) but does not recommend changes or predict outcomes.

SmartFlow is purpose-built for a specific practice profile. The decision framework is straightforward:

- Recovered missed charges. - Technician time savings. Time spent chasing paper treatment sheets, transcribing handwritten notes, and reconciling charges against the invoice is reduced. For a busy hospital, this can save 30–60 minutes per technician per shift.
- Reduced liability risk. Legible, timestamped, standardized anesthetic records and treatment logs are more defensible in malpractice reviews than handwritten paper records.
For practices doing more than $20,000/month in hospitalized patient revenue, SmartFlow typically pays for itself within the first month on recovered charges alone. For smaller practices, the ROI depends more on workflow quality and liability reduction than on direct revenue recovery.
