Neo Veterinary Software Review: Cloud-Based Practice Management for Small Clinics
An independent review of IDEXX Neo veterinary practice management software — cloud architecture, features, pricing, integration limits, and which practices it suits best.
What Neo is and where it fits
Neo is IDEXX Laboratories' cloud-based practice management system, designed for small veterinary practices that want simple, modern software without managing a server. Launched around 2016 and built from the ground up by a veterinarian, Neo focuses on ease of use: one-click consult creation, intuitive scheduling, and a clean interface that new staff can learn quickly.
Within the IDEXX software portfolio, Neo sits below Cornerstone (IDEXX's server-based, full-featured platform) and ezyVet (IDEXX's enterprise cloud system for multi-location and specialty hospitals). Neo is specifically aimed at solo and small practices — typically one to three veterinarians — that want cloud convenience and IDEXX diagnostic integration without the overhead of a more complex system.
How the cloud architecture works
Neo runs entirely in the browser. There is no server to buy, no software to install on local machines, and no IT infrastructure to maintain. The system is accessible from any internet-connected device — desktop, tablet, or smartphone — and data is stored on secure remote servers with 256-bit SSL encryption and automatic backups.
The trade-off is straightforward: you gain anywhere-access and zero IT overhead, but you lose the offline capability that a server-based system like Cornerstone provides. If your internet connection goes down, you cannot access your records until connectivity is restored. For most urban and suburban practices this is a minor risk; for rural practices with unreliable broadband, it is worth weighing carefully.
Core features
Neo covers the essential functions a small practice needs for day-to-day operations:
Practice dashboard. A single-screen overview of daily activity — overdue invoices, reminders due, draft invoices, and scheduled appointments — visible immediately on login.
Patient workflow. One-click access to complete patient history, preformatted exam notes, a master diagnosis list, and low/high estimate templates. Patient records are organized in a linear, timeline-based view.
Scheduling. A simplified appointment calendar that allows staff to view, modify, and start consultations directly from the scheduler. Multi-provider views are available for practices with more than one doctor.
Invoicing and billing. Automated charge capture tied to the patient record. Estimates, invoices, and discharge instructions can be emailed to clients with one click. Digital signature capture is supported on forms, estimates, and certificates.
Client communications. Automated email reminders for appointments, vaccinations, and preventive care. Callback scheduling is built in. Two-way texting is available as an add-on through Vello, IDEXX's client engagement platform.
Reporting. One-click performance reports covering revenue, appointments, client demographics, and practice trends. Reports are less customizable than Cornerstone's but sufficient for small-practice financial monitoring.
AI-powered SOAP transcription. Neo includes an AI transcription feature that can generate structured SOAP note drafts from recorded consultations. This is a relatively recent addition and represents IDEXX's first native AI feature in the Neo platform.
IDEXX diagnostic integration
Neo connects to the IDEXX diagnostic ecosystem through VetConnect PLUS. Practices can order and receive results from IDEXX reference laboratories and in-house analyzers (Catalyst, ProCyte, SediVue) directly within the patient record. Charge capture for lab work is automatic.
This integration is functionally equivalent to what Cornerstone provides and is one of the strongest reasons to choose Neo over a non-IDEXX cloud platform. If your practice already uses IDEXX reference labs or in-house diagnostics, Neo's integration reduces dual entry and transcription errors meaningfully.
What Neo does not include
Understanding what Neo omits is as important as understanding what it provides. Compared with Cornerstone and ezyVet, Neo does not offer:
- Full EMR / medical records management. Patient records exist, but the depth of customizable templates, structured medical history, and documentation tools is limited compared with Cornerstone.
- Native prescription management. There is no built-in prescription tracking or pharmacy workflow module.
- Digital imaging / DICOM integration. Neo does not directly integrate with imaging equipment. Practices that need in-system image viewing will need to pair Neo with an external PACS solution.
- Controlled-substance logging. There is no native controlled-drug tracking. Practices that need DEA-compliant digital logs must use a third-party tool such as VetSnap.
- Inventory management. Neo does not include stock-level tracking, reorder automation, or supplier integration. Practices that want these features must use a standalone inventory platform such as Inventory Ally or rely on manual processes.
- Wellness plans. Petly Plans (IDEXX's wellness-plan product) is not directly integrated into Neo in the same way it is with Cornerstone.
- Online booking (client self-service). Appointment scheduling is internal only. Client-facing online booking is not a native feature.
- Payment processing. Neo includes built-in payment processing with contactless capability in the base subscription, though standard per-transaction processing fees apply.
These gaps are intentional. IDEXX designed Neo to be simple and lightweight for small practices. But they become significant limitations when a practice grows beyond its initial size or adds services that require deeper workflow tools.
Pricing
Neo uses a subscription pricing model. Based on publicly available pricing data from IDEXX and software comparison platforms:
| Component | Cost |
|---|---|
| Monthly subscription (includes first 3 users) | Starting ~$290/month |
| Data conversion (one-time) | ~$590 |
| Setup and training | ~$2,375 |
| Vello client engagement add-on | Additional monthly fee |
| Integrated payments add-on | Additional monthly fee |
IDEXX occasionally offers promotional pricing; the original launch pricing started at $199/month for single-doctor practices. Current published pricing starts higher, and practices should request a formal quote.
For context, a comparable small-practice setup with Cornerstone would cost $6,700–$14,400 annually (including server and IT overhead), while non-IDEXX cloud platforms such as Shepherd charge a flat $299/month regardless of team size.
Where Neo excels
Ease of adoption. Neo was designed to be learned quickly. Built-in training tools, a clean interface, and one-click workflows mean that new staff — including part-time technicians and relief veterinarians — can become productive with minimal onboarding time.
Zero IT infrastructure. No server, no local installs, no backup configuration, no security patching. For a solo practitioner or small team without dedicated IT support, this is a significant operational advantage.
IDEXX diagnostic workflow. The VetConnect PLUS integration works well and saves meaningful time for practices that order IDEXX lab work daily. Lab results flow into the patient record automatically, with charges captured.
Affordability for small practices. At roughly $290/month with three included users, Neo is competitively priced for its target segment — particularly when compared with the total cost of ownership for server-based alternatives.
Cloud accessibility. Access from any device, anywhere, is standard. The practice dashboard and scheduling tools work well on a tablet in the exam room.
Where Neo falls short
Scalability limits. Practices that start on Neo and grow may outgrow it relatively quickly. Adding more veterinarians, expanding services, or needing sophisticated reporting can push a practice toward ezyVet or Cornerstone — which means a second software migration with its own costs and disruption.
Limited third-party integrations. Multiple reviewers note that Neo's integration ecosystem is more restricted than Cornerstone's or ezyVet's. Practices that rely on specific third-party tools — particularly for inventory, controlled-substance logging, or imaging — should confirm compatibility before committing.
No native mobile app. Neo runs in the browser on mobile devices, but there is no dedicated iOS or Android application. For veterinarians who rely heavily on phone-based workflows, the browser experience may feel less responsive than a native app.
Add-on costs accumulate. Features that competing platforms include as standard — two-way texting, payment processing, client engagement tools — are sold separately with Neo. One reviewer reported paying $200+/month for a text-messaging service that competing platforms include at no extra charge.
Inventory gap. The absence of built-in inventory management is a practical limitation. Veterinary inventory management is a significant operational concern — see our guide to veterinary inventory reorder-point systems — and relying on manual processes or a separate platform adds complexity.
Neo vs. other IDEXX platforms
The three IDEXX practice management systems address different segments:
| Factor | Neo | Cornerstone | ezyVet |
|---|---|---|---|
| Deployment | Cloud | Server | Cloud |
| Target practice size | Solo to 3 vets | Multi-doctor, specialty | Multi-location, enterprise |
| Feature depth | Essential only | Comprehensive | Comprehensive |
| Customization | Limited | Extensive | Moderate to extensive |
| AI features | AI SOAP transcription | None native | None native |
| Inventory management | Not included | Built in | Built in |
| Controlled-substance logging | Not included | Built in | Built in |
| Pricing model | Subscription (per practice) | License + annual support | Subscription (per provider) |
| Offline capability | No | Yes | Limited |
Practices evaluating Neo should also consider whether they may need Cornerstone- or ezyVet-level features within the next three to five years. If growth is likely, starting on a more capable platform may avoid a costly second migration.
When Neo is the right choice
Neo fits practices where:
- The practice has one to three veterinarians and no plans for rapid expansion.
- IDEXX diagnostics are the primary lab partner and VetConnect PLUS integration is a priority.
- The practice owner wants zero IT overhead and prefers a simple, cloud-native tool.
- Staff turnover or frequent relief-vet usage makes quick onboarding important.
- Budget is a primary concern and the total cost of ownership needs to stay predictable.
When to look elsewhere
Consider alternatives when:
- You need built-in inventory management, controlled-substance logging, or DICOM imaging integration. Cornerstone or ezyVet include these natively.
- You plan to grow beyond three doctors or add a second location within a few years. Migration costs compound.
- You want built-in AI documentation beyond basic transcription. Shepherd and Digitail offer more mature AI SOAP tools.
- You need deep third-party integrations beyond the IDEXX ecosystem. Confirm that your current tools connect to Neo before committing.
What to ask during a demo
If you are evaluating Neo, ask your IDEXX representative:
- What is the all-in monthly cost — subscription, Vello, payments, and any other add-ons — for a practice your size?
- What does the data conversion process look like from your current system, and what is the typical timeline?
- If you outgrow Neo, what does migration to ezyVet or Cornerstone involve? Are there published migration paths?
- What third-party integrations are currently live, and which are on the roadmap?
- How does the AI SOAP transcription feature work in practice, and is there an additional cost?
