Revolution Plus for Cats: What Topical Preventive Covers, What It Does Not, When It Fits
Revolution Plus (selamectin + sarolaner) is a monthly topical for cats that covers fleas, ticks, heartworm, ear mites, roundworms, and hookworms.
What Revolution Plus is
Revolution Plus is a prescription monthly topical solution for cats and kittens 8 weeks of age and older weighing at least 2.8 lb. It combines two active ingredients:
- Selamectin — a macrocyclic lactone that prevents heartworm disease and treats fleas, ear mites, roundworms (Toxocara cati), and hookworms (Ancylostoma tubaeforme).
- Sarolaner — an isoxazoline that adds flea-killing power and extends coverage to four species of ticks.
The solution comes in single-dose tubes applied to the skin at the base of the cat's neck. Three weight ranges are available: 2.8–5.5 lb, 5.6–11 lb, and 11.1–22 lb.

What Revolution Plus does not cover
- Tapeworms other than flea tapeworm — it does not treat Taenia species or Echinococcus. If a cat hunts or has access to rodents, a separate tapeworm treatment such as praziquantel may be needed.
- Adult heartworms — Revolution Plus prevents infection; it does not treat existing adult heartworm disease.
- Coccidia, giardia, toxoplasma — none of these protozoal parasites are on the label.
- Mange mites (Notoedres, Demodex) — not labeled for cats with these conditions (unlike in dogs, where some avermectin products are used off-label).
- Dogs — Revolution Plus is labeled for cats only. Dog households that need similar broad-spectrum coverage use different products.

The primary clinical reason to choose Revolution Plus over Revolution is tick exposure. If a cat lives in or travels through tick-endemic regions, or if the household wants the broadest single-tube coverage, Revolution Plus adds that layer.
Safety considerations
Isoxazoline class warning
Sarolaner is a member of the isoxazoline class. The FDA has issued a fact sheet warning that isoxazoline products have been associated with neurologic adverse reactions — including tremors, ataxia, and seizures — in some dogs and cats, even in animals without a prior history of neurologic disorders. Revolution Plus carries this warning on its label.
In practice, the reported rate of neurologic events in cats receiving Revolution Plus is low. In a margin-of-safety study, one cat at five times the recommended dose experienced piloerection, tremors, and dilated pupils approximately 24 hours after the third monthly dose; signs resolved without treatment within two hours, and the cat completed the study without further issues.
Use with caution in cats with a history of seizures or neurologic disease. If your cat has ever had a seizure, tremor, or unexplained collapse, tell your veterinarian before starting Revolution Plus so the risk-benefit can be discussed.
Common side effects
In clinical field studies, adverse effects in 1–10% of treated cats included:
- Lethargy
- Decreased appetite
- Hair loss or stiff hair at the application site
- Itching not at the application site
- Conjunctivitis
- Sneezing
These effects are typically mild and self-limiting.
Not evaluated
- Kittens under 8 weeks or weighing less than 2.8 lb.
- Breeding, pregnant, or lactating cats — safety has not been established. Selamectin alone has been tested in reproducing cats, but the combination with sarolaner has not.
Bathing
Bathing the cat two hours or more after application does not reduce the effectiveness of selamectin against heartworm. However, label data on the effectiveness of the full combination after bathing have not been evaluated — so do not bathe the cat on application day.
When Revolution Plus is a good fit
- Indoor-outdoor cats in tick-endemic areas who need the widest parasite coverage from a single monthly dose.
- Multi-cat households where topical application is easier than pilling, and one product can cover fleas, ticks, heartworm, ear mites, and intestinal worms.
- Cats with flea allergy dermatitis — the dual-action flea killing may offer faster relief than selamectin alone.
- Cats over 22 lb — the largest tube covers up to 22 lb; Zoetis notes that a second appropriate-size dose can be used for heavier cats. Discuss the correct total dose with your veterinarian.

What to ask your veterinarian
- Does my cat's lifestyle actually require tick protection, or would Revolution (without sarolaner) be sufficient?
- Has my cat been tested for heartworm? (Testing is recommended before starting any heartworm preventive in cats over 6 months old.)
- Are there other pets in the household that need their parasite prevention coordinated?
- My cat has had [seizures / tremors / neurologic signs] in the past — is an isoxazoline product appropriate?
- What is the plan if my cat has an adverse reaction after application?
